As the Account Holder, I am initiating the transfer of my current Internet Services to the new address detailed above. I acknowledge that my preferred dates and times for this move are subject to availability, and I will be informed of the confirmed schedule through both SMS and email. I understand that in the event the assigned date and time are inconvenient, it is my responsibility to contact Wakey Internet immediately to arrange an alternative schedule. I am also aware that my existing services will cease at my previous address and will commence at the new location as per the agreed schedule. Furthermore, I commit to relocating the existing equipment to my new address. This includes, but is not limited to, the modem, router, and all requisite cables, ensuring a seamless transition of service. I acknowledge the imposition of a $49 fee associated with this service relocation and hereby express my consent to proceed with the Move of Service under these terms.
